An 8-segment prologue plan: save Esme, rescue Lazar, find Claw Art bracers
A conservative prologue route with a spare segment, the correct medicine, the crypt bracers, and clear trade-offs for reading and treasure hunting.
Your best prologue reward is a useful set of equipment and the people you meant to save still alive. Make a manual save outside the family home. Check the hourglass cost before each choice; the route below leaves one segment for a detour instead of promising every activity.
Read the two time warnings
An hourglass is an action cost. A clock beside a journal entry warns that the quest is time-sensitive. Walking, inspecting the map, and ordinary looting do not consume segments simply because you spend real time on them. Some marked activities resolve their cost automatically, however: stop and check when the time bar flashes.
Avoid training skills with an hourglass cost during the prologue. Treasure that remains available afterward is a poor reason to miss a time-sensitive rescue.
A route with one segment in reserve
This is a conservative adaptation of Doms Roundtable's quest tests, not a promise to reproduce his character level or loot. The core actions budget seven segments.

Use the spare segment for Page-Turner if you want time with Anca. If you already spent it on play, training, or another encounter, prioritize delivering the medicine. Keep your original save rather than overwriting it to force this route to fit.
Deep Down: collect the bracers before leaving
Follow the search for Lazar from Gavril, using Focus to pick up the trail. Doms explicitly shows that arriving too late can leave Lazar dead. Do this before clearing every treasure site in the woods. See the rescue discussion at 08:04.
Take the torch near the cave entrance and quickslot it. In the chamber with wall pictures and a sarcophagus, inspect the pictures until the coffin becomes interactable. Collect Mireleaf Bracers, continue to the guardian encounter, loot the enemy, then escort Lazar home.
If the coffin will not open, check the inspectable wall clues. If Lazar is already dead, restoring the rescue outcome requires an earlier save.
Esme needs hot water and three scoops
Buying the herbs is only half the job. Return home, choose hot water, add three scoops, and finish administering the medicine. IGN's first-steps guide and Doms at 13:03 agree on this combination.
Paying Anca more and staying to read are separate choices from treating Esme correctly. If you are short on time, secure the treatment first.
Why the medicine matters — outcome spoiler
Doms compares the Mass scenes after successful treatment and failed treatment. Esme's fate changes; this is not merely a different experience reward.
Two investigations worth finishing carefully
For Blasphemy, inspect the footprints, loom, and cloth around Gramla's workspace, then confront Premysl. If your goal is helping Gramla, retrieve the banner and report back. This route does not depend on the video's alternate double-reward sequence.
For Disturbed, follow the grave clues. Near the armorer, head west through the alley, inspect the shovel to the right, and enter the house to progress the investigation. The additional reburial offer spends more time. Your position in the family dispute is a role-playing choice, not a mandatory experience optimization.
Recover when your schedule slips
With two segments left and no herbs, go directly to Anca and then home. Read a newly found Broad Swing manual, but postpone training it. Leave the bear treasure until after the prologue. Do not grind to match a video's displayed level: its fights and random drops may differ from yours.
Continue with your first open-world night or the early legendary route.
